Tolworth schoolboy died from heart attack after an "unexplained" accident inquest concluded
Schoolboy Samuel Orola died when he suffered a heart attack after an accident in a school playground, an inquest has concluded.
The five-year-old collapsed in the adventure playground in Tolworth Infants School during lunch break on Thursday, September 15, last year.
A jury at South West London Coroner’s Court has this morning concluded that Samuel suffered a vagal cardiac arrest after an accident of an "uncertain nature."
The only witness to the devastating scene was another five-year old schoolboy, who cannot be named.
The schoolboy suggested Samuel, who was believed to be playing on a climbing frame, may have done a roly poly, a headstand and fallen from a bench, before collapsing, the inquest heard.
His father who gave witness at the start of the inquest on Wednesday, May 30, said: "Our understanding is he (Samuel) had done a roly poly on the A frame then went towards the benches.
“He was being wobbly, going from bench to bench, fell off the bench and hit his head on a plant thing."
The inquest also heard two supervisors were overseeing about 30 Year 1 children in the adventure playground, which consisted of slides, monkey bars, a climbing frame, a tower and a tunnel.
Head of play safety for the Royal Society for the Prevention of Accidents David Yearly said he inspected the playground on September 20, last year, and found it fit for purpose.
Coroner Sarah Ormond-Walshe said it was a “very difficult situation” and understood it could be “a little difficult” to piece together the events leading up to Samuel’s death.
She said: “We have a situation, looking at the photographs, where it may be that Samuel had been using this A frame; he may have done a roly poly on the A frame and [it appears] that afterwards he seems to have walked a few steps to the bench ....this is where the person who spoke to the paramedic said he just sat on the bench feeling unwell, got up [and] fell to the floor.
“It might be that the little boy who was playing with Samuel at the time thinks in the collapse there may have been a handstand. It’s a difficult situation.”
Verdict: Accidental death.
